No referral is needed for most insurance plans — you can book online directly. Some HMO plans require a referral from your primary care physician; call us at (713) 353-3699 and we'll help you check.
We accept all major Medicare, Medicare Advantage, and commercial insurance plans, and we're happy to assess workers' compensation and personal injury patients. If you don't see your plan listed, call us — we can usually help.
A photo ID, your insurance card, a list of current medications, and any recent imaging (MRI, CT, X-ray) reports or discs. You'll receive paperless intake forms by email and text before your visit.
Dr. Iyer builds multidisciplinary plans that emphasize treating the source of pain — with injections, nerve-targeted procedures, physical therapy, and non-opioid medication. When opioids are appropriate, they're prescribed thoughtfully and monitored carefully as one part of a broader plan.

Parking is free on the second floor of our building at 5373 W Alabama St. Take the parking ramp off Yorktown Street up to the second-floor structure and walk straight into the clinic.
Procedures are performed with local anesthetic and image guidance, and most take under 20 minutes. Patients typically describe pressure rather than pain, and sedation is available when appropriate.
